Terms of Use
These terms explain the responsibilities and limitations connected with using The Ames Foundation Home Health Monitoring Program.
1. Agreement to these terms
By registering for or using the platform, you agree to these Terms of Use, the Privacy Notice, and the Participant Consent.
2. Who may use the platform
The standard participant registration process is intended for adults age 18 or older unless The Ames Foundation has created a separate youth-participation process.
3. Educational purpose
The platform provides health education and self-monitoring tools. It does not provide medical advice, diagnosis, emergency monitoring, or treatment.
4. Your responsibilities
When using the platform, you agree to:
- Provide information you reasonably believe is accurate
- Use only your own participant record
- Protect your participant ID and account access
- Follow the instructions provided with your home devices
- Contact your healthcare team about health concerns
- Avoid attempting to view or change another participant’s information
- Avoid interfering with the security or operation of the platform
5. Medications and treatment
Do not change medication, treatment, diet, fluid intake, or monitoring frequency based only on information displayed in the platform.
6. Platform availability
The Ames Foundation does not guarantee uninterrupted or error-free access. The platform may be changed, temporarily unavailable, or suspended for maintenance, technical, security, legal, or program reasons.
7. External links
The platform may link to public-health and educational websites operated by other organizations. The Ames Foundation does not control those websites or their privacy practices.
8. CDC information
Educational content may be informed by resources from the Centers for Disease Control and Prevention.
The Ames Foundation and the Home Health Monitoring Program are not affiliated with, sponsored by, certified by, or endorsed by the CDC.
9. Limits of the platform
The platform is intended to support education and self-monitoring. It cannot guarantee that it will identify, prevent, diagnose, or treat a medical condition.
10. Changes to these terms
The Ames Foundation may update these Terms of Use. When important changes are made, participants may be asked to review and accept the updated terms before continuing.
