Program purpose
The Home Health Monitoring Program helps participants record basic health measurements at home, learn about their numbers, and share information with program staff when requested. Participation is voluntary.
What participation involves
Participants may record measurements such as blood pressure, blood glucose, weight, and symptom notes. Authorized staff create a KYN Participant ID privately after eligibility screening and informed consent. The public registration form creates independent tracking IDs only. Independent users are not enrolled in the pilot and are excluded from pilot reporting.
Voluntary participation
Joining the program is voluntary. You may stop participating at any time by discontinuing use of the app.
Potential benefits
Benefits may include better awareness of trends in your measurements and improved self-management.
Risks and limitations
This platform is intended for education and self-monitoring. It does not provide medical advice, diagnosis, or emergency monitoring. Data entry errors or device inaccuracies are possible.
Data collection and use
We collect limited data to support the program (see the Privacy Notice). Program participant data may be used for program delivery, participant support, quality improvement, and aggregate reporting. The restricted crosswalk linking an Inquiry ID to a KYN Participant ID is maintained separately by authorized staff.
Stopping participation
You may stop using the platform at any time. Contact program staff if you need help withdrawing.
Not a replacement for professional care
This platform does not replace care from a qualified healthcare professional. If you have health concerns, contact your care team.

If you believe you are experiencing a medical emergency, call 911.
