This notice explains what information the Home Health Monitoring Program collects, why it is collected, and the choices available to participants.

The Ames Foundation values your privacy and is committed to handling participant information responsibly.
2. Information we collect
We may collect information you choose to provide, including:
- Participant ID
- A protected hash of your PIN (the PIN itself is not stored)
- Optional email address
- ZIP Code, city, or county
- Age range and sex
- Health conditions you choose to identify
- Blood pressure and blood sugar readings
- Weight
- Medication-adherence responses
- Symptoms and urine-protein screening results
- Notes or other information you voluntarily enter
- Dates and times connected with registration and check-ins
3. Why we collect it
We collect information to:
- Support your home self-monitoring
- Show your previous measurements and trends
- Provide health education
- Manage and evaluate the program
- Improve future community-health programs
- Meet grant, reporting, audit, or legal requirements
4. How information may be used
Authorized program staff may use your information for program support, evaluation, security, troubleshooting, and improvement.
The Ames Foundation may combine participant information into grouped reports for grants, program evaluation, community reporting, and program improvement. These reports are intended not to identify individual participants.
5. How information may be shared
The Ames Foundation does not sell participant information.
Information may be shared:
- With authorized program personnel
- With technology providers that help operate and secure the platform
- When you direct or authorize the disclosure
- When required by law or valid legal process
- To investigate fraud, misuse, or a security incident
6. How we protect information
The Ames Foundation uses reasonable administrative and technical safeguards designed to protect participant information. Access is limited to authorized personnel who need the information for approved program activities.
No website, database, or electronic transmission system can be guaranteed to be completely secure. Protect your participant ID, PIN, device, email account, and login information.
7. How long information may be kept
Information may be kept for as long as reasonably needed to operate and evaluate the program, meet reporting obligations, maintain required records, resolve concerns, and comply with the law.
8. Your choices and privacy requests
You may contact The Ames Foundation to request:
- Access to information connected with your participant record
- Correction of inaccurate demographic information
- Deletion of information when allowed
- Withdrawal from future program participation
- An explanation of how your information is used
We may need enough information to confirm that the request belongs to you. Some records may need to be kept for legal, security, grant, audit, or recordkeeping purposes.
9. Children
The standard registration process is intended for adults age 18 or older. Do not register a child unless The Ames Foundation has created a separate youth-participation process with appropriate permission from a parent or legal guardian.
10. Changes to this notice
This notice may be updated when the program, technology, or legal requirements change. A new effective date will be displayed when the notice is updated.
